Impose class limitations for commonly abused classes like sniper, spy, sniper and snipers. How about a map won't start till there is 1 medic for every 6 players? This alone will improve the quality of the game by orders of magnitude. To the OP, "Where are the good pubservers at?", the one where spammah is playing obviously. P.S. I miss #5 and #6 too. If you hate map cycle and only want to play on a specific map like Qj pointed out Badwater. There is! Can't find the steam group. But its one of the most populated server in asia.
